Care Coordination: The intersection of primary care, oncology, and the patient from initial diagnosis to survivorship

This course will discuss how to assist the patient with coordination of care in the cancer patient. How to schedule testing, planning and also community resources will be discussed. Appointments and referrals can be a difficult task to coordinate. This speaker will discuss how to assist the patient on making this process easier and less stressful.

Lauren Cook

RN

Lauren Cook graduated from Catawba Valley Community College with an associate degree in Nursing in 2010. She then attended Appalachian State University to obtain a Bachelor of Science in Nursing. Lauren has almost 15 years of nursing experience, all of which has been in Hematology and Oncology in the inpatient and outpatient setting. She has held her Oncology Nurse Certification through ONS since 2017. She is currently Ambulatory Nurse Manager at a satellite Hematology and Oncology clinic with a larger Comprehensive Cancer Center, where she has worked for the last ten years. She has a special interest in care of the rural oncology patient, as it is often complex and challenging. Most recently, she participated in a project assessing barriers to clinical trial access for rural patients and this project was presented at ONS Congress in April of 2024. She is married and has two children. Her interests outside of work include being active in her church, exercise, reading, and baking cookies.
