This will be an uplifting, hands-on participation activity where we will discuss practical and holistic ways to recognize and reduce polypharmacy in the geriatric population, including shared decision-making and the importance of brown bag medication reconciliation when we have limited time with our patients. We will then evaluate and discuss practical strategies to improve medication review and adherence. We will briefly review beers criteria, and I will provide you with other practical references for deprescribing.

Michelle Wade is a dual-certified Nurse Practitioner specializing in Adult-Gerontological Primary and Acute Care. She is a Fellow of the American Association of Nurse Practitioners (AANP) and a passionate advocate for nurse practitioners and patients at both the state and national levels. Currently, she serves as the President of the Vermont Nurse Practitioners Association and is actively involved in the Conference and Policy Committees. Additionally, Michelle is a board member of the American Nurses Association of Vermont and a graduate of the original AANP Leadership Program. She is currently participating in the AANP Practice Committee.
As an educator, Michelle received the W. Robert Wonkka Advising Award at Vermont Technical College, her alma mater, where she served as a full-time faculty member. She continues to teach as needed across the curriculum at Vermont State University, from the Practical Nursing (PN) program to the Bachelor of Science in Nursing (BSN) program, and enjoys mentoring future nurses. Michelle is a founding member of Omega Mu Sigma and maintains an active membership in Omicron Delta. She founded the college’s chapter of the Alpha Delta Nu honor society and was named Elsevier’s National Educator of the Year during her full-time tenure.
In her clinical role as the lead hospitalist at Gifford Medical Center in Randolph, VT, she provides comprehensive care to a diverse rural population at a critical access hospital. Michelle precepts nurse practitioner students and encourages them to stay and practice in Vermont. She has contributed to an acute care nurse practitioner textbook, reviewed clinical guideline texts, and presented nationally on various topics.
In recognition of her numerous projects and dedication to the profession, Michelle was honored with the inaugural AANP State Award for Outstanding Contribution in 2025. Additionally, in 2018, she received the AANP State Award for Clinical Excellence in Vermont for leading a successful initiative that reduced hospital readmissions from local nursing homes by 70% in one year.
