Overview

The LGBTQIA+ population has evolved over the past several years to include other gender non-conforming and gender minority populations. The individual needs of each population within this acronym vary and as a result need to have specialized, focused care in order to meet their healthcare needs. These gender and sexual minorities still encounter issues concerning sexuality, identity, gender, and freedom of expression with their healthcare providers. It is a well-documented fact that the majority of healthcare providers today have not been properly educated on how to care for this population therefore, the specific needs pertaining to each individual group represented in this acronym go largely ignored. This session will focus on how to approach the care of LGBTQIA+ patients and address their specific needs in an effort to promote patient-centered care.

Overview

Building upon the revised diagnostic framework of the DSM-5, this presentation defines somatic symptom disorders, reducing the focus upon lacking medical explanations for physical symptoms in order to make the diagnosis of “medically unexplained symptoms.” With the renewed goal of increasing patient access to appropriate treatment, symptoms that are substantially more severe than expected in association with distress and impairment are emphasized. The presentation explores conditions with no medical explanation and conditions where there is some underlying pathology but an exaggerated response. These concepts are operationalized to enhance the nurse practitioner’s skill in assessing, diagnosing, and managing these challenging disorders in a variety of settings.

Abstract

The treatment of patients with diabetes mellitus is a balance between good glycemic control and the risk of hypoglycemia. Fluctuations in the blood glucose level are common, but are often missed with fingerstick blood glucose monitoring. This limitation can be overcome with continuous glucose monitoring systems. Challenges interpreting the large amount of data provided by CGM systems have limited their clinical application. Recent guidelines on CGM and the use of the ambulatory glucose profile to interpret the data are helpful in simplifying the use of CGM systems in clinical practice with the benefit of reducing fluctuations in the blood glucose level.TBD

  • Identify patients who are better candidates for continuous glucose monitoring versus fingerstick blood glucose monitoring.
  • List the types of information provided by continuous glucose monitoring systems.

  • Modify the treatment plan based on CGM data to improve patient outcomes.

Abstract

Dementia is a common disorder affecting nearly eight million people in the United States. Although there are various causes of dementia, neuropsychiatric symptoms are a common feature, with nearly all older adults with dementia experiencing one or more neuropsychiatric symptom. Among these neuropsychiatric symptoms, delusions occur in an estimated 25% and hallucinations in up to 1-in-5. Delusions and hallucinations appear to not only increase the risk of aggressive behavior, but they are also associated with worsened outcomes. As the majority of this large patient population of some 2,400,000 receive ongoing care from a primary care provider (PCP), the education of those providers in the recognition and treatment of this condition is critical.

Learning Objectives

 At the end of the activity, participants will be able to:

  • Identify the burden experienced by patients with dementia-related delusions and hallucinations.
  • Assess patients with dementia for the presence of delusions and hallucinations.
  • Individualize treatment in patients with dementia-related delusions and hallucinations.
  • Align treatment of patients with Parkinson’s psychosis with current recommendations.

Overview

Abstract

The management of patients with suspected chronic kidney disease (CKD) involves numerous clinical decisions beginning with considerations for screening and extending through optimal treatment. This activity explores many of these clinical questions encountered by primary care clinicians, with a discussion about 2020 guidelines issued by Kidney Disease – Improving Global Outcomes (KDIGO) and the American Diabetes Association (ADA). While renin-angiotensin-aldosterone (RAAS) inhibitor therapy is well established in the treatment of patients with CKD, the sodium glucose cotransporter-2 (SGLT-2) inhibitors are quickly assuming an important role in these patients, with as well as without diabetes mellitus. Clinical questions related to the use of the SGLT-2 inhibitors in patients with CKD are discussed amidst the growing evidence supporting their important role.

Learning Objectives

At the end of the activity, participants will be able to:

  • Identify the risks and consequences of chronic kidney disease.
  • Screen appropriately for the presence of CKD in patients at high risk.
  • Describe the safety and efficacy of SGLT2 inhibitors on renal outcomes in patients with and without type 2 diabetes mellitus.
  • Initiate evidence-based therapy for lowering blood glucose in patients with established kidney disease as recommended in updated guidelines.
