Palliative Care: Impacting Outcomes in Primary Care

Back to Package

Palliative medicine is a relatively new specialty. It is important that advance care planning take place before the patient faces a serious illness or medical emergency. Primary care is the best place for these conversations, due to the relationship between a patient and their primary care provider. Utilizing case studies, you will learn more about the 5 Wishes form, a legal document in almost all US states that can be filled out and completed as a patient's advance care plan.
